GRADES OF PRESSURE ULCERS
Pressure ulcers can readily change from just a reddened area on the surface of the skin to a deep sore or ulcer extending down through the skin into the tissue beneath the skin and even down to the bone. Pressure ulcers are classified or graded in the following manner.
Grade 1 - Redness (reddened area that cannot be blanched)
a. Pinkish, red or blotchy skin that may be associated with irregular swelling or heat over a bony area, for example, a heel or elbow, not resolving after 15-20 minutes. You will need to stay off the area for 1-2 days until the redness resolves.
b. The soft tissue is inflamed (red/swollen).
c. Continued pressure results in breakdown or a Grade 2 ulcer.
d. If appropriate pressure relief is given, a Grade 1 pressure ulcer will heal without any further treatment.
Grade 2 - An Actual Break in the Skin
a. There is breakdown through the top skin layers. The skin appears cracked, broken or blistered.
b. There is shallow to full-thickness skin injury. Breakdown is observed on the skin, not only redness.
c. Progression to this stage must be prevented. A scar will result with healing. Remember that scar tissue is more likely to breakdown again.
d. You will need to keep pressure off the area for an extended period of time and you may have to do regular dressing changes.
Grade 3 - The Fat Layer is Visible
a. This is a full-thickness ulcer which extends to the fat layer.
b. Swelling of muscle occurs.
c. There is fluid and protein loss from the ulcer. Drainage is visible.
d. There may be swelling of nearby joints.
e. Hospitalization with intense nursing care and bed rest may be required. Grade 3 sores may require surgery for closure.
Grade 4 - Bone is visible - This Ulcer Can Be Deadly!
a. All top skin layers are necrosed (dead.) Bone can be seen.
b. Prolonged hospitalization (3 months or more).
c. Surgery with intensive nursing care and antibiotic therapy required.
d. Condition may be fatal, may lead to sepsis (blood poisoning).

درجات قروح الجلد

ج- ضغط مستمر ينتج عنه إصابة بالجلد أو قرحة من الدرجة الثانية.
د- إذا تم تخفيف الضغط على نحو ملائم فإن قرحة الضغط من الدرجة الأول سوف تشفى دون أي علاج إضافي.
الدرجة (2) - قطع فعلي بالجلد
أ- توجد قروح خلال الطبقات العليا من الجلد. يكون شكل الجلد فيها مشقوق أو مفتوح أو متقرح.
ب- توجد منطقة ضحلة تصل إلى إصابة بكل سمك الجلد. تلاحظ القروح بالجلد بالإضافة إلى الاحمرار.
ج- يلزم إيقاف استمرار التقرح عند هذه المرحلة. تظهر ندبة مصحوبة بالتئام. تذكر أن النسيج الندبي يمكن على الأرجح أن يصاب بقرحة مرة ثانية.
د- ينبغي عليك تجنب وقوع ضغط بالمنطقة الملتهبة لفترة ممتدة من الوقت وقد يلزم عليك تغيير الضمادات على نحو منتظم.
الدرجة (3) – تكون فيها طبقة الجلد واضحة
أ- يحدث تقرح بكل سمك الجلد حيث يمتد إلى الطبقة الدهنية.
ب- يحدث انتفاخ للعضلة.
ج- يخرج سائل من القرحة وفقدان للبروتين.
يكون تصريف السائل واضح.
د- قد يوجد انتفاخ بالأربطة المجاورة.
هـ- قد تتطلب هذه الدرجة علاج بالمستشفى مع رعاية طبية مركزة وراحة بالفراش. قروح الدرجة الثالثة قد تتطلب جراحة للالتئام.
الدرجة (4) – يكون فيها العظم واضح - يمكن أن تكون القرحة مميتة بهذه المرحلة!
أ- تكون كل طبقات الجلد العلوية متنخرة (ميتة). يمكن رؤية العظام.
ب- تتطلب علاج ممتد بالمستشفى يمتد إلى (ثلاثة شهور أو أكثر).
ج- تتطلب جراحة مع رعاية طبية مركزة وعلاج بالمضادات الحيوية.
د- قد تكون الحالة مميتة، وقد تؤدي إلى تعفن (تسمم الدم).

Both parties hereto declare that they are legally competent to enter into this contract and hereby agree as follows:
Preamble

Street, and is desirous to sell the same to the second party, and whereas the second party is desirous to buy the above mentioned villa for use, and accepts the purchase.
