Hi,
I'm proofreading a medical report.
This is the page on social security.
"Informe COMPLEMENTARIO a cumplimentar con el tercer parte (dia 18 de la BAJA) y sucesivamente cada 4 partes"

"SUPPLEMENTARY report to be filled in together with the third report (day 18 of the sick leave period), and thereon after every 4 reports"
