Primary care is a term used for the activity of a health care provider who acts as a first point of consultation for all patients, e.g. a general practitioner (GP).

In some parts of the world, patients cannot directly consult a specialist but must be referred to the specialist by a primary care practitioner.
However, "primary care patients" does not necessarily mean "patients on first consultation".

The main point is that the meaning "primary" as a first point of consultation does not mean "first consultation", although it is very often for primary care patients to visit a primary care physician on their first consultation.
As what I have previously mentioned, in some medical systems, patients cannot directly visit a specialist on their first consultation but must be referred to the specialist (e.g. a cardiologist) by visiting the primary care physician on their first consultation.
This situation is different in Taiwan where patients can directly visit a specialist (e.g. in the department of psychiatry) or a secondary care practitioner without first visiting a primary care physician or a general practitioner.

: All right, Shirley. I see the point now. It is the first consultation, not the first clinic visit. However, 初诊 may refer to the first health care consultation, too, as it can be verified by googling. Some hits strongly hint the possibility.
